Community Palliative Care Covid-19 Response by Guildford and Waverley Integrated Care Partnership

Community Palliative Care Covid-19 Response by Guildford and Waverley Integrated Care Partnership

South East

Building on a history of strong partnership working, during the pandemic Guildford and Waverley ICP’s end of life care team acted swiftly to ensure individuals and their loved ones continued to receive the best possible care and support, despite increasing pressure on local services.

Their collaborative response brought together specialist palliative care providers, community services, general practice, the out-of-hours provider, the ambulance service, the voluntary sector and social care, working alongside the local acute hospital, supported by Surrey Heartlands CCG.

Nominated by: Rt Hon Jeremy Hunt MP
